how to create hyperlink where users can add title and a link to a URL

I want to create a web application where users can add title with textbox to enter detailed description and a field to enter the source of information(URL). The title should be a hyperlink to the given URL. Which should be visible to the members of the website only after it has been approved by admin. Somewhat like how we can see in stackoverflow. How do I go about it?
